Exams for United States Green Cards or US Citizenship

We perform IMMIGRATION physical examinations!

Medical examinations are required by the USCIS (United States Citizenship and Immigration Services) to obtain a United States green card or United States citizenship. Only doctors designated by the USCIS as civil surgeons can complete an immigration exam.

The purpose of an immigration exam is to determine whether you are eligible for admission to the United States on public health and safety grounds.

SPFMC have been providing immigration physicals for more than fifteen years. We provide all the necessary medical services to fulfill immigration exam requirements for US citizenship applicants of ALL ages.

SPFMC also provides vaccination assessments and services for refugees.

The government requires completion of form I-693 in order to fulfill your immigration physical requirements.

What Does an Immigration Examination Include?

Physical Examination
Review of medical, mental and behavioral history
Review of vaccinations
Screening for tuberculosis, syphilis, and gonorrhea
Completion of all exam results reporting, including the I-693 form
Tuberculosis screening is done using a blood test called Quantiferon-TB GoldSyphilis Test (Required for all applicants age 18-44, or as determined necessary by the civil surgeon)Gonorrhea Test (Required for all applicants age 18-24, or as determined necessary by the civil surgeon)Additional fees will apply for any vaccines that are needed.

What To Bring To Your Scheduled Appointment:

A valid government issued photo ID (passport, Driver's license, ID card, Permit to work, etc). If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent's full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
The latest version of I-693 Form with pages 1-3, part 1 through part 4, filled out. DO NOT sign page 2 part 2 of the form.
Official documents of vaccination or immunication records (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease, and COVID-19)
List of medications
Any past medical records of previous tuberculosis testing and/or treatment, including chest x-ray reports or other imaging reports.
An interpreter who can speak, read and write English if needed
Payment (needed at the time of visit). Usually health insurance will not cover immigration examinations.

What to expect during the immigration exam:

The immigration medical examination entails a review of your medical history and a physical examination.
The civil surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination. The civil surgeon will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC). CDC sets the requirements for the immigration medical examination in the Technical Instructions for Civil Surgeons | CDC.
Once all your test results are received by our office, the civil surgeon will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. It may take anywhere from 7-14 days to receive test results back.
You will get a copy of the completed Form I-693 for your personal records before the civil surgeon seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.
